DESCRIPTION:The objective was to create a computerised ‘Learning Healthca
 re System’ to improve the safety of patients with AKI. A Learning Health
 care System is where outputs are automatically fed back into the system 
 to generate continuous improvement and innovation. AKI is a good patient
  safety target because it moves away from tackling individual diseases\,
  allowing wider safety issues to be highlighted\, such as polypharmacy a
 nd safer transitions of care\, particularly for vulnerable patient group
 s.\n\nWe produced a prototype learning healthcare system and used mixed 
 methods to investigate the data and usability requirements of the system
 . Six indicators of quality for AKI care was embedded into the system. A
 ll users felt the system was useful in highlighting patient safety impro
 vement opportunities particularly in missed chances for uncoded AKI pati
 ents. The system learnt to prioritise suggested actions due to user feed
 back\, to speed up actions and processes and in turn also generated lear
 ning for users.\n\nLearning Healthcare Systems provide potential to impr
 ove care. However\, due to competing resources\, making a significant cl
 inical change requires user engagement and presence of an adequate champ
 ion reinforced by organisational support
