End of life care has for some time been recognised as a crucially important aspect of quality healthcare service provision, and in the last decade has become increasingly prominent in both political discourse and the mainstream media. However quality of end of life care continues to vary greatly across the UK, due to a complex range of factors.
Based on a mixed-methods study of End of Life Care conducted across three regions in the North of England, this paper first discusses the use of Death in Usual Place of Residence (DiUPR) as a proxy for high-quality, choice-led end of life care, then moves to an analysis of the critical success factors which shaped the differing performance of the three boroughs in this study in relation to DiUPR.
Discussion focuses in particular on three key themes; the contribution of district nurses in delivering co-ordinated care, the policy and practices of acute trusts, and the impact of General Practitioner engagement and performance. In conclusion, generalisable lessons are drawn out for improved measurement and performance in relation to choice-led and integrated end of life care.
